What Medicare Part D is, in plain English

Part D is prescription drug coverage, run by private insurance companies under Medicare’s rules. You can get it two ways: as a stand-alone drug plan that sits next to Original Medicare (usually alongside a Medigap policy), or built into a Medicare Advantage plan. Each plan sets its own monthly premium, its own list of covered drugs (the formulary), and its own tiers and copays.

How the costs work in 2026

The old “donut hole” coverage gap is gone, and there’s now a hard ceiling on what you can pay out of pocket for covered drugs in a year:

  • Deductible. You pay full price until you meet the plan’s deductible — no more than $615 in 2026.
  • The annual out-of-pocket cap. Once your out-of-pocket spending on covered drugs reaches $2,100 in 2026, you pay $0 for covered drugs for the rest of the year.
  • Spreading the cost. You can also choose to spread your out-of-pocket drug costs across the year in level monthly payments rather than paying them all at the pharmacy.

The penalty for waiting

Part D is optional — until it isn’t. If you go 63 days or more without creditable drug coverage after your Initial Enrollment Period, you can owe a late-enrollment penalty that’s added to your premium for as long as you have Medicare drug coverage. Even if you take only one prescription today, signing up on time is usually the cheaper long-term move.

How to pick a plan in Canton

The plan with the lowest premium is rarely the cheapest plan for you — what matters is how your specific drugs land on each plan’s formulary and tiers.
